WellClix Announces Chronic Care Partnership with Pine Lodge Assisted Living

Henderson, Texas— October 2, 2023 — Today at Pine Lodge Assisted Living, WellClix announced the implementation of Chronic Care Management services, enabling residents to receive chronic care management within the comfort of their home within the Pine Lodge community.

Resident Impact

Older adults are affected by chronic conditions, such as diabetes, arthritis, and heart disease. Nearly 95% have at least one chronic condition, and nearly 80% of have two or more. WellClix and WindRiver, owner of Pine Lodge Assisted Living, are committed to deploying Chronic Care Management services to all their residents to improve quality of life.

Chronic Care Management

Chronic care management includes a comprehensive care plan that lists health problems and goals, providers, medications, community services that patients have and may need moving forward, and other health information. It also explains the care needed and how providers will coordinate it.

“Our providers work alongside the primary care team and with direct care staff to prepare an individualized care plan for each resident,” said Dr. Jason Miller, psychiatrist, and family medicine physician. “Our goal is to support residents’ chronic conditions through medication management, and urgent care needs to enhance medical support and offerings within the community.”
